Instructions for Garage Mail Audits

General Information

The information at the top of page one of the forms contains specific information concerning your policy, such as:

Policy Number: Your Garage Liability policy number
Agent: Your agent's information
Policy Term: The period this policy was in effect. When reporting the requested information, please use this time period.

Please complete the worksheet that comes with the mail audit packet.

Dealers: You are a dealer if you are in the business of buying and selling automobiles and have dealer plates. To complete the form:

1. List all your employees by name in the first column. If anyone is provided a dealer automobile for their use, please indicate this by their name.
2. Next, show their dates of employment for the policy term. If they worked the entire policy term, you can indicate "full year."
3. In the "Total Weeks Worked During Policy Period" column, enter the total number of weeks they actually worked for you during the policy term. Any part of a week is considered a week. In addition, please indicate the type of duties performed (salesperson; clerical office; lot, etc.)
4. It is not necessary for you to enter the total wages each employee earned during the policy term in the last column. Please leave this area blank. (This was required on for other garage type policies).
5. In the "Are You Operating As" section, indicate your business structure (Individual; Partnership; Corporation).
6. Complete the Owners, Partners, Officers section, listing each owner or officer, their title and duties.
7. Complete the "Audit information provided by:" section.

Your mail audit report is now complete. Please return this form by fax to Safeco at (425) 556-4067 or by mail using the enclosed pre-addressed envelope. If at any time you need assistance. Please call us at 1-800-422-6340 or contact your agent.
